What is Bridges to Excellence Program?

An initiative by Health Care Incentives Improvement Institute (HCI3), the BTE program recognizes and rewards clinicians who deliver superior care to patients with chronic conditions. BTE programs measure the quality of care delivered through certain outcomes and process benchmarks.

Most health plans have adopted BTE programs as part of their pay-per-performance initiatives.

Why should I join BTE?

The benefits of participating in the program fall into two categories:

  • Alignment with national programs

Many BTE measures and definitions align with programs such as CMS' Physician Quality Reporting System (PQRS)

  • Financial incentives and preferred network status

Once you are BTE-recognized, you may be eligible to earn a fixed annual bonus payment for each patient who is a member of a participating employer or health plan.

Other rewards may include preferred network tiering and fee schedule increases.

Who can participate?

Clinicians get recognized and gain a wide range of incentives and benefits.

  • BTE Recognition proves that they deliver a superior level of patient care.
  • Highlighted entries in plan directories and websites.
  • Preferred network tiering.
  • Potential fee schedule increases.
  • Fixed annual incentive bonus payments.

Receive high quality care from the top clinicians in the area.

  • The BTE collaboration among employers, health plans and clinicians is designed to spur continuous improvements in the quality of healthcare and put patients at the center.
  • As a participant in BTE, they can research the quality of a specific clinician or practice, and find qualified clinicians in their area.
  • These clinicians meet measures of care developed by organizations like the American Diabetes Association, American Heart Association, American Stroke Association, and others.

How do I get started?

HCI3 outlined these three main steps to start you off on participating in the BTE program:

Step 2: Determine which performance assessment pathway is best for you.

There are several pathways to submit clinical data to a performance assessment organization (PAO), depending on the program that you want to be recognized for.

One pathway is through automated EMR/Registry System that is HCI3 established. Meditab is a certified EMR vendor who has partnered with HCI3 for diabetes and asthma care recognitions. Other methods include NCQA's Diabetes Recognition Program and Medconcert.

Meditab submits your clinical data through CeCity Medconcert.

Step 3: Apply for recognition.

Now that you've chosen an option for data submission, you can now apply for a BTE recognition through Meditab. Contact your account manager for more information on how to apply.
